Telehealth Visit Login Instructions
New Patient Telehealth Visit Instructions:
- You will be sent a link to your email to access and set up your patient portal
- Upon completing your log in you will need to go the “My Health” tab to complete your patient information
- Remember to “Save and continue” at the bottom of the page on each tab
- Email (firstname.lastname@example.org) or Text (716-970-4140) your I.D card and insurance card (front and back) to the office
If you are not able to complete your health information via the patient portal please obtain the New Patient Intake form on our website located under the patient portal tab. You may send the completed form to us via email or take a photo of the paperwork and text it to the office.
***Your patient portal needs to be completed OR send completed intake form to the office BEFORE your first visit.
Follow up Teleheath Visit:
- Please inform the office if you have any changes to your basic information- (insurance, pharmacy, address) etc.
